A soft-bound booklet measuring 7.5 x 10 cm when closed. The first interior page shows an image of the medal in color and serial number of the certificate. On the opposite side is the name of the award recipient appears to be Seongcheol Jang (please note, we are not at all certain about the accuracy of the translation), born in September 1949 in the Pyongan Province. The award was bestowed upon him based on a 19 September 1958 decree.
