A booklet with red rubberized cover, approx. 7 x 10 cm, same format as similar Soviet medal documents of the period. The citation states that the award was issued based on a 15 May 1988 decree of the President of Afghanistan. The typographically printed text is in Russian and Pashto, while the hand-written entries are in Russian only.

Very good to excellent condition. The cover is completely free of wear or soiling. The internal pages have a couple of very minor stains that are not detractive; some of the ink has smeared when the document was filled-out, but all the entries are still fully legible.
