In booklet format measuring 4" x 2 ½", in hard covers bound in high-quality red leatherette with the embossed image of the badge on the front cover, in gold paint. The "citation" page states the serial number of the document, full name of the awardee, the seal of the RSFSR Ministry of Education, facsimile signature of the Minister of Education, and hand signature of the Chairman of the RSFSR Teachers Trade Union. The rest of the pages list the eligibility requirements for the award.

In excellent condition. The golden luster of the badge on the front cover has faded but the paint is intact, and the badge is clearly identifiable. The internal pages are clean, the printed and handwritten text, seal and signatures are clear and perfectly legible.
