Hard-bound booklet wrapped in maroon leatherette, measuring approx. 6.5 x 9 cm when closed. The front cover has an impressed depiction of the badge and gold inscription "Ministry of Healthcare of the USSR". The first internal page shows the hand-written name of the recipient, date of the award decree, and the 2 June 1971 date when the document was filled-out (one day after the decree.) Hand-signed by the Chief of the Healthcare Ministry Personnel Directorate.

Excellent condition. The cover shows practically no wear; the binding is tight. The internal pages are perfectly clean and crisp.
