This post-Soviet issue document still shows the state emblem of the USSR on the cover, but it is of noticeably different format than the far more common documents for the same medal from the 1940s-50s. The most noticeable difference of course is that it does not have a depiction of the medal on the first inside page. The document is signed by the Deputy Head of the Administration of the Moscow Region and shows the appropriate regional administration seal with the state emblem of the Russian Federation. The back cover has a stamp of the Social Care Directorate, town of Khimki (in Moscow region) and a hand-written note that an ID allowing special privileges was issued on 26 April 1995 (the day after the issue date of the medal document). This scarce certificate would be a perfect addition to a Variation 3 "Voenkomat" medal.

In very good to excellent condition. There are a couple of very small and faint stains on the cover and first inside page, none of them very detractive. The document is otherwise clean and perfectly sound.
