In booklet format measuring 6.5" x 5" when closed. Luxury hard cover with beveled boards wrapped in high-quality leatherette. The lettering on the front cover is in gilt, featuring the state emblem of the Ukrainian SSR, followed by "Certificate of Induction into the Honor Roll of the Ministry of Internal Affairs of the UkrSSR".

Inside, the left page specifies the name of the inductee, and the reason for the induction: "for many years of conscientious service in the UVD (Russian abbreviation for Office of Internal Affairs) of the Chernigov Region".

The right page states Order # 512 of 18 November 1971, and bears the seal of the Presidium of the Supreme Soviet of the USSR and the name of the minister, Col. Gen. I. Gladush.

In excellent, outstanding condition. The leatherette of the cover is very clean and shows only minuscule wear to the corners. The gilt of the lettering has retained all of its golden luster. All the printed and handwritten text inside is perfectly clear and legible, and so it the seal.
