Singe page document, front and back, mostly in two sizes, 6" x 6" and 6" x 8". Filled out by hand on special stationary of the Narkomat of Defense, this one-page letter was sent to the local military commissariat. The commissariat was then to notify the closest family member of the fallen serviceman, as shown in the document, nearly always his wife or mother.
