The ID, issued on 9 May 1919, certifies that Leontiy Gumenchuk serves as a musician with the aforementioned brigade and has the right to bear a firearm. The ID is valid until 1 June 1919.

In very good to excellent condition. The paper is remarkably clean and pliable; there is mild wrinkling, mostly to the right side, but no fraying to the edges. The left margin shows four tiny holes from filing staples, not very intrusive
