The document was issued by the Committee of the Red Partisans of the Tomakivksiy District (part of the Ekaterinoslav / Dnepropetrovsk Region of Ukraine). It includes a photo of the recipient wearing a typical holiday attire of an industrial worker or farmhand. The name of the recipient appears to be A. P. Perezanov (А. П. Перезанов); the handwriting of the last name however is sloppy and not clearly legible, so it's partly a guess on our side.

The document is in good condition. The cloth binding shows wear but remains sound; the image is faded but still discernible. The interior likewise shows wear but is free of significant soling or stains. The text is mostly legible Like many documents of this type, the ID has remnants of a strip of light-green paper where it was apparently attached to an archival file.
