The medallion is in outstanding, excellent condition. The details are perfect and exceptionally crisp on both sides. The obverse shows no wear visible to the naked eye, while the reverse has only a few minuscule dings that are nearly unnoticeable. There is an attractive mild patina to the brass with the original "gilt" mint luster beautifully preserved in recessed areas throughout.

The medal comes on an original suspension, a WW2 two-layer model in brass with self-locking pin. The nicely preserved ribbon is old, possibly of the period. The connecting link appears to be of the period. To summarize, this is a truly superb example of the Defense of Caucasus Medal.
