The obverse features Semashko's portrait in sculpted relief, his name and initials, and the mark of the Leningrad Mint at about 4 o'clock. The reverse shows a caduceus and the words "The First Narkom of Public Health", followed by the year's of Semashko's life, 1874 - 1949. The sculptor's name, A. Korolyuk (A. Королюк) is at the very bottom of the reverse, followed by the year of the medal's issue.

In excellent condition, no significant wear or damage of any kind visible to the naked eye. An occasional tiny spot of verdigris can be seen in recessed areas at magnification.
