Similar in format to a WW2 era order booklet, hard covered with red cloth binding. The document was filled-out on 9 August 1976; it shows the 28 July date of the award decree and a stamp of the Presidium of the Supreme Soviet of Ukrainian SSR and facsimile signature of its chairman.

In very good to excellent condition. The cloth binding has a small tear on the spine (visible only on the verso) and a very tiny spot on the front that may brush off, otherwise perfect. The internal pages are immaculately clean and crisp.
