In excellent condition. The enamel on the suspension is completely free of the usual chips, flakes or significant wear of any kind, and has only a microscopic contact marks completely unnoticeable without magnification. This is an extremely unusual case for such an early award. The medallion is practically pristine, with crisp and perfect details. Shows very attractive patina to silver on both sides.

The document is an early type order booklet filled-out on 1 March 1946. Note that unlike the later issues, the printed text refers specifically to the republic where the award was bestowed - in this case, Uzbekistan. Shows that the award was bestowed by a decree of the Supreme Soviet of Uzbek SSR on 27 February 1946 - very soon after the introduction of the motherhood decorations in 1944.

The document is in fair to good condition. There are stains to the cover and internal pages, but the cover doesn't show excessive wear. The binding is tight; all the pages are present and crisp. Despite the stains, all the text is completely legible.
