The certificate is a hard 10.5 x 7.5 cm booklet wrapped in red cloth, similar to an order booklet. It was filled-out on 3 February 1957 and shows no other awards. There is an ink stamp "valid without a photograph" in the space provided for the photo.

In excellent condition. The cloth binding is tight and nearly pristine, completely free of soiling or significant wear. The internal pages are crisp and immaculately clean with the exception of the usual rust run-off from the staples.
