Missing In Action notice, showing name and rank of the soldier, year of birth, and date when the soldier went missing in action.
Singe page document, front and back, mostly in two sizes, 6" x 6" and 6" x 8". Filled out by hand on special stationary of the Narkomat of Defense, this one-page letter was sent to the local military commissariat. The commissariat was then to notify the closest family member of the missing serviceman, as shown in the document, nearly always his wife or mother.
