In very good to excellent condition. The form was properly de-stapled when removed from its original file, preserving the filing margins, and leaving only barely noticeable staple holes. The paper is very slightly wrinkled, mostly at the top of the pages. The blue ink blots in the same area are small, at the edge and do not affect the typed text. Both pages share a very small rip in the middle of the bottom edge. Both signatures, as well as the typed text, are clear and perfectly legible. The paper is lightly age-toned.
