The document is of very early type, printed in 1945 and different from the motherhood medal certificates issued in the 1950s and later. Note in particular the soft cloth bound-cover unlike the hard cover of the later documents. The document was definitely awarded with one of the Variation 1 medals featuring early style of the suspension device. The document was filled out on 17 April 1946. It shows the date of the award decree and a stamp of the Presidium of the Supreme Soviet of RSFSR.
